DR. GENTILE’S PRACTICE POLICIES

San Francisco Psychotherapist Dr. Amy Gentile PoliciesBILLING: You will pay for sessions weekly, unless another arrangement is agreed upon. Fees will be discussed during initial contact. Private pay and insurance reimbursement are accepted.

INSURANCE: For contracted insurance agreements that I have accepted below, you are responsible for your co-pay weekly and I will bill your insurance company. Insurance benefits (such as deductibles, copayments, and number of sessions) vary according to plan and employer. You are responsible for finding out your plan benefits and fees and keeping track of any changes.

CANCELLATION POLICY: Once we have agreed upon a regular time or times to meet during the week, I will reserve that time(s) for you unless you cancel 48 hours in advance. If I am able to schedule another appointment during your time (s) or if we can reschedule for another time that week, I will not charge you. Please note insurance companies do not pay for missed sessions.

TELEPHONE: I available for emergencies 24 hours a day. Otherwise, you can leave me a message at 510-654-5146 and I will return your call within 24 hours.
My emergency contact number is available on my message machine for your reference 510-599-2399.

VACATIONS: When I am on vacation or otherwise unavailable for appointments, I will leave the name and number of someone to call in the event of any emergency, or a number where I can be reached.

CONFIDENTIALITY: All information is held strictly confidential unless:

  1. The patient authorizes release of information with his/her signature.
  2. The patient presents a physical danger to self.
  3. The patient presents a physical danger to others.
  4. Child/elder abuse/neglect are suspected.
